Hackensack Meridian Health and Cadence partner to transform senior care

First-of-its-kind remote care delivery system will extend the reach of clinicians and future-proof care for New Jersey’s seniors

EDISON, N.J. – October 21, 2025 – Hackensack Meridian Health, New Jersey’s largest and most comprehensive health network, and Cadence, the nation’s leading remote care delivery system, today announced a strategic partnership to build the infrastructure to deliver proactive senior care. By extending care beyond clinic walls through connected devices and Cadence’s AI-powered platform, Hackensack Meridian Health will help seniors stay healthier, more independent, and always connected to their care teams.

Through the partnership, Hackensack Meridian Health patients use connected devices at home to share vital signs such as blood pressure, weight, heart rate, and blood glucose. Cadence’s AI-powered Proactive Care Engine analyzes this data in real time, alerting the 24/7 clinical team when intervention is needed and ensuring patients receive support with medications, and lifestyle guidance.

For clinicians, the program functions as a seamless extension of their practice. Cadence’s clinical team handles daily monitoring, follow-through and patient outreach, while intelligent alerts and care gaps surface cases that require immediate attention. This approach reduces administrative burden, helps prevent burnout and allows Hackensack Meridian Health providers to focus their time on the patients who need them most. The program is already monitoring more than 1,000 patients.

Cadence’s programs have delivered measurable results for partner health systems nationwide, including:

  • 70% relative increase in patients achieving guideline-recommended blood pressure goals
  • A 230% increase in heart failure patients on all four pillars of guideline-directed medical therapy)
  • $1,300 average annual savings per patient
  • 27% lower hospitalization rate at 12 months post-enrollment
